If I need to get a Med Waiver to re-enlist will it automatically make my PUHLES score go down to a 2?

I need to get a Med Waiver for Air Guard for my shoulder. Will that automatically make the 'U' on my PUHLES score go down to a 2? Because I thought you couldn't enlist if you had a bodily deformity that didn't relate to eyesight or hearing...the reason I ask is because I want to do Fire Protection which requires a score of 111221. Thanks a lot!
